So far this is the only method working consistently for me as I also did not want to install various 'mods'. Figuring the small size of the first save should get things running, I tried that. I read some forum posts that part of the problem is the ballooning size of the save file, among other suggestions. My current thought is the game may still be loading background stuff which causes a load glitch if I try and start the game before it is ready. New theory: I have noticed if I wait during the intro screens until the music comes in strong (all the horns and base sound) and then wait until shortly after the background switches, I seem to be loading any save game OK. It has also failed to load a couple of times even using the small save game to start. After the game starts, I open the Load menu again and load my current game. Infinite loading screen work around for me: I choose to load the first save game I ever made (in Goodsprings) which is a very small file. I guess its because of the changes that they have made with other games to make them more stable, that I expected the same treatment for this game.
Naturally you can install other mods on top of these, assuming they're compatible - there are specific YUP patch-projects to ensure YUP can be used with other mods, and AFAIK the anti-crash fix is compatible with all other mods. Squid830: I realise it's probably not what you want to hear, but if you want to know the best way to get NV to work well. pretty much exactly as it was released (barring bug-fixes to those things). Note that installing only the above will ensure you can play NV with a minimum of crashes and other bugs, while keeping the core gameplay, mechanics, quests etc.
Unfortunately I can't see GOG fixing this, unless of course by "fixing it" one means "comes pre-installed with the above" - which is possible, since they did it with Vampire: The Masquerade: Bloodlines (albeit that only required one patch). YUP : Fixes a vast array of bugs without changing content or gameplay (IMO this is essential) For me it still crashes occasionally, but nowhere near as often - and I can't remember if I even ever had the never-ending loading bug. New Vegas Anti Crash: This should stop most crashes.
Fallout Mod Manager: For painless mod management. New Vegas Script Extender: Needed for a lot of mods, including some of the below. There are thousands of bugs left in every TES and Fallout games which are fixed by modders.
